Netflix’s teen mega-hit Outer Banks has cheerfully shunned realism in favor of anything eyebrow-raising, including car chases, stolen goods, secret trysts, and fugitive escapes, ever since its pandemic-era apex in 2020. However, the drama’s second season, which debuted on July 30, shockingly raised the stakes. We’re obviously referring to Sarah Cameron’s death, which made the dubious miraculous healings on Grey’s Anatomy appear like kid stuff.
In the opening moments of OBX season 2, Sarah (Madelyn Cline) is shot in the side by her elder brother Rafe while she and John B (Chase Stokes) are attempting to flee in a truck loaded with $400 million in gold. Since they are fugitives and are stuck in Nassau, John B rushes Sarah to the house of Captain Terrance’s favorite doctor.
The only things we know about this doctor are that he “doesn’t do heads,” he has a charming small clinic on the seaside, and he allegedly found a treatment for arthritis when he was still in medical school. (Good thing Sarah wasn’t hit in the face with a bullet!) Also, he’s, er, “not a doctor.” In his own terms, he acts “more like a service provider.” Naturally, while your girlfriend is bleeding on the table in front of you, that is precisely what you want to hear.

The unfortunate kid is charged $4,000 for surgery in which the unqualified surgeon a) performs on Sarah in the kitchen, b) doesn’t wear a mask, and c) immediately after stitching up Sarah’s incision, sips from a “salty dog.” They wait for hours as Sarah briefly regains consciousness to say “hello” to John B before suddenly going into cardiac arrest. Let’s examine the specifics of what takes place next.
Did Sarah Cameron Die on “Outer Banks”
Technically speaking, yes. John B conducts frantic CPR as Mr. Arthritis Cure embodies the human equivalent of the shrugging emoji when her heart stops. The display flatlines behind them, and he informs John B, “She’s just lost too much blood, kid.” While her lover yells at her to wake up, Sarah is technically still unconscious for a number of minutes. Her fingers don’t twitch until he confesses his love to her, at which point the Sleeping Beauty kiss of life begins to work its magic.
Not-Doc enters the room once more, notices that she is still alive, and nods to John B as if to say, “Good work on that love confession! It worked wonderfully!” Actually, no. The research, however, reveals that there is a distinction between “clinical death” (when the heart stops beating) and biological death (actual, irreversible death).
The Lazarus phenomenon is a delayed return to life after resuscitation and is named after a biblical figure who comes back to life four days after his demise. A heart might technically cease and then restart several minutes later.

But Healthline notes that this is a rather uncommon occurrence: Only 32 instances of the effect between 1982 and 2008 were discovered by one 2015 investigation. It is very unlikely that Sarah would have felt it just after John B told her “I love you.” Even less likely is the scenario in which she would be mobile hours later without obvious brain impairment or walking difficulties.

We observe Ward and Rafe driving off with the gold valued at $500 million as John B and Sarah are in the Bahamas. A trap set up by Terrance and Cleo sees a group of kids leap out to slash the truck’s tires just as the victims believe they are safe.
Without a car to flee in, Ward and Rafe continue before being restrained by Terrance and Cleo. Rafe opens fire on John B and Sarah when they arrive at the scene during the holdup and are moving near the truck. Sarah admits that she has been shot before John B is able to drive off with her.
